Week 6 Unit 3

As I hadn't complete the whole section of '2a collect information' I worked towards completing this and meeting the milestones in the space of this week although I haven't finished it yet, as I've been busy and creating the form took some time.

So, I had created my online form (the method I had chosen) in Dreamweaver; Website software. This was easy to use as I used it unit 2 but I still had to familiarise myself with the software itself for awhile. I then used the different features within the software to create my form. This can be found on the right. This is the form placed on the internet as evident from the web address. 

milestone 5 (collect the information): I had achieved this as first of all, I had created my method which is the form in this case. I have also received the information that is shown as follows on the excel file;

I had collected this onto this excel file for now and then I will transfer it to a database and review the information thoroughly (the next milestone). 

Collecting the information wasn't difficult, setting up the form was. I had to add a number of boxes and format them appropriately. I had resolved this problem by looking at other sample forms and basing my form around them - in a unique way however so it was different.
